Testimonial Consent

I, hereby, agree and acknowledge as follows.

1. I am aged 18 years or over.

2. I give Integria Healthcare (Australia) Pty Ltd (Integria) permission to use and reproduce the whole or part of my testimonial for advertising and promotional purposes [together with or without my name, in its publications, marketing and on its websites, and in such position as Integria may determine in its’ sole discretion.

3. I authorise the use and reproduction of my testimonial in various formats including online advertising, electronic and hard copy publications, Integria-related websites and social media sites and in any other similar ways deemed appropriate by Integria without compensation to me.

4. The opinions and views stated by me in the testimonial are genuinely held by me.

5. All statements regarding my use, or other experience of, the named product are correct and fairly represent my use and experience.
